Documents required for admission

Before you come for admission, please read the following carefully and bring the documents listed.

General surgery and medical treatment

• The patient's photo ID.
• The treating physician's admission order.
• Medical records supporting the operation, such as ultrasound, X-ray, laboratory, MRI and CT scan reports.

Surgery for patients under 18

• The father must be present at admission.
• If the father cannot attend, the child's legal guardian must be present. In order of priority, the legal guardian is:
  1. the father, or the father's notarised consent;
  2. the paternal grandfather;
  3. the mother, if she is divorced and holds the custody document;
  4. any person who has custody of the child and brings the custody document.

Obstetric and gynaecological surgery, curettage and termination of pregnancy

• Birth certificates (shenasnameh) and ID cards of both spouses.
• The treating physician's admission order.
• The husband must be present.
• For curettage and termination of pregnancy, authorisation from the university of medical sciences and from the court is required, together with the husband's presence or his notarised consent.

Cosmetic surgery

• Married women must be accompanied by their husband; this does not apply to divorced women or to single women over 18.
• For patients under 18, the rules for children apply.
• Photo ID of the father or legal guardian, and the child's ID.
• Admission order stamped and signed by the treating physician.
• Medical records supporting the operation, such as ultrasound, X-ray, laboratory, MRI and CT scan reports.
