With warm greetings: to help improve quality and advance the hospital, we would be most grateful if you would kindly answer each of the questionnaires below.
Patient Safety Culture Assessment Questionnaire
This questionnaire has been prepared with MS Forms and is highly secure (both for you and for us). To answer it, simply:
- Select “Patient Safety Culture Assessment Questionnaire”.
- Answer the questions on the page that opens.
- When you have answered the questions, select Submit at the bottom of the page to send your answers.
- After you submit, a new page opens confirming that your answers have been sent.
Please note that your answers will reach us anonymously.
If you wish, after submitting you can save a copy of your answers to “your own Microsoft account” for your personal records (the application gives us no way to store your answers, and we cannot save “your answers”; it only sends us a “copy” of your answers).
To save your answers to “your own Microsoft account”, select the Save my response to edit button on the “response submitted” confirmation page; the application will sign you in to your Microsoft account and save your answers.
As well as keeping your answers in your personal records, this also lets you change your answers later and send them to us again anonymously (as long as the questionnaire is still open for responses and has not passed its closing date).
